Testing Rescheduling

This form is not for a first time request. To schedule a test, please visit our test scheduling form.

All rescheduled exams require prior written approval by your instructor/professor.  If your professor does not provide the approval and/or you have not made the appropriate pre-arrangements with SLS, you will not be able to take your examination in the Office of Student Learning Services.

If you must reschedule an examination during finals week, you must come to the Office of Student Learning Services for discussion of if and how this may impact other examinations during the week.   Tests will not be rescheduled with SLS until one-on-one communication between SLS and the student has been addressed and written authorization has been received from the instructor.
